MAKING AN APPOINTMENT TO ESTABLISH CARE AT IRIDESCENT WELLNESS CLINIC for patients ages 12 and older:
If you are experiencing symptoms of an urgent nature, please call 911 or go to the nearest emergency room immediately.

Requests for appointments are monitored weekdays (excluding Monday) from 8 am to 4:30 pm. You will be contacted within two business days of leaving your request. Telemedicine appointments are offered and you are encouraged to utilize this option if you are feeling unwell and to limit potential COVID exposure.
